COVID-19: Visitation Fact Sheet

  1. Visiting shall be:
    • Monday through Friday from 11:00 AM TO 3:30 PM by appointment only
    • Limited to 30 minutes
    • Held in outdoor areas (weather permitting) and indoor (when outdoor visitation is not an option) in a well-ventilated space with no more than ten (10) individuals in the room.
    • Limited to family members, loved ones, and representatives from the long-term care ombudsman program (LTCOP) and resident advocacy organizations
    • Strictly prohibited on resident floors, resident rooms and care areas except for Compassionate Care visits.
    • Limited to two (2) visitors per resident per day
  2. Visitors must wear a face mask or face covering that covers the nose and mouth when on the premises.
  3. Residents, visitors, representatives of the long-term care ombudsman and resident advocacy organizations will be required to appropriately use an alcohol-based hand sanitizer, consisting of at least 60 percent (60%) alcohol, provided by the Facility, prior to visiting the residents.
  4. Visitors must maintain social distancing. Touching permitted if resident is vaccinated.
  5. The Facility will screen visitors for signs and symptoms of COVID-19 prior to resident access and will prohibit visitor(s) exhibiting any COVID-19 symptoms or that do not pass the screening questions. Screening shall consist of both temperature checks and asking screening questions to assess potential exposure to COVID-19 which shall include questions regarding domestic or international travel (
  6. Visitors will be encouraged to bring a negative COVID-19 test result taken within 72 hours of the visit. Point of Care testing is available for visitors who are unable to provide a test.
  7. The following information must be provided for each visitor: first and last name of the visitor; physical (street) address of the visitor; daytime and evening telephone number; date and time of visit; and email address, if available. All visits must be scheduled through the Recreation Therapy Department.
  8. Compassionate Care visits are available and can be arranged by contacting the Social Work Department at 718-206-5119.
  9. Visitors under the age of 16 will not be permitted. Visitors under the age of 18 must be accompanied by an adult 18 years of age or older.
  10. A copy of the Facility’s formal visitation plan is available for review at any time. The Visitation Policy will be posted on the facility’s website.
  11. The staff is required to monitor the visitation process. In accordance with the NYSDOH March 25, 2021 Health Advisory, if any visitor fails to adhere to the Facility’s visitation plan, he/she/they will be prohibited from visiting for the duration of the COVID-19 state declared public health emergency.
